How can a Specialist help me?

Here at 32Co, we’ve built the first ever platform to give dentists the ability to collaborate with a Specialist Orthodontist on every case- and we are proud of it!! Working with a colleague when you begin providing clear aligner therapy makes sure you feel confident delivering safe and effective care.


To show you, here’s a sneak peak of some examples of what you can expect when working with our orthodontists.

No magic system or algorithm

There is no magic system behind successful clear aligner treatment, it’s built from a combination of:

- Rigorous clinical assessment

- Proper diagnosis

- Comprehensive dentist/orthodontist-driven design (not solely a technician!)

- Effective delivery and monitoring

We know that orthodontic treatment starts with a proper and accurate assessment. Incorrect diagnosis = incorrect expectations and the incorrect outcome! Sometimes a case can have complex features which can be easy to overlook or difficult to spot.


Specialists offer clear advice about when and why a case is complex. This case-based approach to education helps to develop your orthodontic knowledge as you go. Understanding the risks and decision process behind your treatment options will help you feel in control when choosing which cases to take on. 


Here’s a real example of a conversation between a GDP on our platform and one of our specialists:

“Thank you for submitting this case. This is a very difficult case due to the following:

- Almost 100% OB

- Spacing in the lower arch - which can further deepen OB if closed

- Retroclined ULS

I would not recommend taking on this case with clear aligners because:

- Correcting the retroclination requires creating palatal root torque on the upper incisors which is very difficult to achieve with clear aligners

- A deep overbite can be challenging with clear aligners

- Closing the space with clear aligners will further deepen the bite.


If the patient would like the teeth aligned only, then this is something we can do, but you would have to accept the retroclined incisors and OB. I’ve sent you a paper that summarises tooth movements that are particularly challenging to achieve with clear aligners – hopefully you find it interesting!”

Support improving your orthodontic knowledge / skill level 

Often there are multiple ways of approaching a case, and selecting which one is right will depend on the patient's treatment goals and your level of experience. Every case is an opportunity to learn and develop so our Specialists help to direct you to educational content that is tailored to you.


Example:

"Thank you for submitting this case. The patient has an increased OB and OJ. If you would like this corrected then we would need to utilise class 2 elastics. Please let me know if you would like:

1) alignment only

2) correction of OB/OJ with cl 2 elastics


I can see you haven’t used elastics on a case with 32Co before. This could be a good opportunity for you to do this. I’ve sent an elastics guide to you in the portal and linked you to a very similar case study you might find useful."

Comprehensive treatment designs and advice from an Orthodontist.

Our specialists create comprehensive and in-depth treatment designs with you to ensure you and your patient get the best outcomes. They use all the data gathered about the patient to formulate their design – so it’s important the case work up is as complete and accurate as possible.

"Aims of treatment for this case: 1) Align and level upper and lower arches, 2) Improve the anterior functional guidance, 3) Widen the smile, 4) Maintain overall Class I occlusion.

Upper arch:

  1. Gain space for alignment with some anterior IPR and arch expansion. Expand the arch using lingual root torque/inclination at the upper canines and premolars bilaterally. This will create space within the arch, allowing for alignment without gum recession.
  1. Procline the upper centrals, with some lingual root torque/inclination
  1. Retract the upper lateral incisors into the line of the arch, after initial mesiopalatal rotation to align
  1. Using the created space, move the upper midline to the patient's left to match the lower midline

Lower arch:

  1. Gain space for alignment with IPR and mild arch expansion. Additional lingual root torque/inclination to be placed at all lower premolars.
  1. Derotate the lower central incisors, by rotating the mesial aspect labially.
  1. Do not procline the lower lateral incisors, use their position as a reference for the final position of the lower central incisors.
  1. Maintain the lower midline in its current position."

Isn’t all this the same as popular providers?

❌ No! The treatment plans with these traditional manufacturers are created by technicians, not dentists and definitely not orthodontists.

❌ No! They do not assess complexity for you or behave as colleagues, they are a manufacturer and they follow your instructions.

❌ No! They do not always protect you from taking on complex cases which clear aligners may struggle to correct. 

❌ No! They do not help you with patient work up, consent and ongoing management
